Checklist of Required Reports

You may chek the status of your online reports by clicking on the Report Status button.
After you login, click the Chapters button on the left, then the Report Status button.
You will find current status and real-time deadlines.
Report Deadline
Fall Officer Information Update Submit online by October 31
Officer Election Report Submit online within 2 weeks after election
Officer Signatures Postmark October 31 or if there is a change in officers
IRS Form 990-N 15th day of 5th month after close of fiscal year
Report of Eligibility Submit online as soon as list is available (within first five weeks of term)
Report of Election & Catalog Cards Submit Report online as soon as electees accept (at least two weeks before initiation). Mail Catalog Cards
Report of Final Action & Roll Book Signatures Submit Report online immediately after initiation. Mail Rollbook signatures
Initiation fees Mail to HQ within 2 weeks after initiation
Bottom portion of Convention Delegate Credentials Due to HQ within 2 weeks after election of delegate
Top portion of Convention Delegate Credentials Bring to Convention
Convention Delegate Expense Report Postmark by deadline on expense report
Constitutional Amendment Ratification Ballot Postmark by April 1 (no extensions)
Listing Eligible Curricula in Bylaws Certain chapters only - was due by April 1, 2004.
Financial Report Submit online by June 1
Chapter Survey Submit online by June 1
Project Reports Postmark by June 1
